Data Collection Form
Participating states are asked to review past projects that have experienced premature pavement deterioration or construction related materials problems. A database will be created for these projects and the research team will attempt to determine what testing could have identified the problem prior to construction.
The states will be asked to furnish what materials and construction information
may be available. They will outline what testing was done to determine the
cause and what their conclusions were. In addition, they will be asked about
changes in procedures or specifications they may have implemented to address
the problem that occurred.
